Specific educational objectives are as follows: Program Objectives (Based on ACGME Core Competencies) 1) Patient Care: Trainees must be able to provide patient care that is appropriate and effective for the treatment of Malawis common health problems (malaria, HIV/AIDS, malnutrition, parasitic infection, diabetes, hypertension). Residents are expected to: communicate effectively with PIH staff, patients, and their families (with use of a translator); gather essential and accurate information about their patients; make informed decisions about diagnostic and therapeutic interventions based on available resources at PIH and available resources within local referral centers; develop and carry out patient management plans and counsel and educate patients and their families (with use of a translator); understand how to support patient care decisions and patient education in a setting of limited information resources and information technology; perform competently all medical procedures considered essential for the area of practice; provide health care services aimed at preventing health problems in the setting of limited health prevention services, and to provide patient-focused care. 2) Medical Knowledge: Trainees must demonstrate knowledge about established and evolving biomedical, clinical, epidemiological, and social-behavioral sciences and the application of this knowledge to patient care. Residents are expected to demonstrate analytic thinking and apply the basic and clinically supportive sciences to the decision making process. 3) Interpersonal and Communication Skills: Trainees must be able to demonstrate interpersonal and communication skills that result in effective information exchange with patients, families, and professional colleagues. In certain cases information exchange will require a translator. Residents are expected to elicit and provide information using effective nonverbal, explanatory, and questioning techniques. They will also demonstrate effective written communication (in patient passports and clinical charts) and will be expected to work effectively with others as a member of a diverse health care team. 4) Professionalism: Trainees must demonstrate a commitment to carrying out professional responsibilities, adherence to ethical principles, and sensitivity to a diverse patient population. Trainees are expected to: demonstrate respect, compassion, and integrity; a responsiveness to the needs of patients and cultural issues specific to the Malawian setting; and a commitment to excellence and on-going professional development. Trainees must also demonstrate a commitment to ethical principles pertaining to confidentiality of patient information, informed consent, and sensitivity and responsiveness to patients culture, age, and gender. 5) Systems-Based Practice: Trainees must demonstrate an awareness of and responsiveness to the larger context and system of health care in Malawi. They should: learn how medical practice in Malawi differs from that of UCLA private and public institutions, including methods of practicing within extremely limited resources; identify ways to assist patients in dealing with complex medical problems in the setting of limited diagnostics and referral systems; learn how to partner with local Malawian health care providers and public health entities (government and non-government) to assess, coordinate, and improve health care systems in Malawi. 6) Practice-Based Learning and Improvement: Trainees must be able to investigate and evaluate their patient care practices, assimilate scientific evidence, and improve their patient care practices based on this type of self-assessment. Specifically, residents are expected to: analyze practice experience and perform practice-based improvement activities using a systematic methodology appropriate to the Malawian setting; locate, appraise, and assimilate evidence from scientific studies related to their Malawian health problems; apply knowledge of study designs and statistical methods to the appraisal of clinical studies and other information on diagnostic and therapeutic effectiveness relevant to the Malawian setting; use limited information technology in Malawi to manage information and access on-line medical information; and support their own education; facilitate the learning of US and Malawian students and other health care professionals.
